Please Note:  Prior to downloading the application ,please take some time and look at our Mission Statement, Philosophy of Christian Education, and our Statements of Faith and Intent.  These will help you to understand what ECA is all about. 

Application Steps

Before an interview can be completed, the following steps must be taken adn forms must be turned into the office:

1. Read all materials.  If you have any questions or concerns, please do not hesitate to contact us.

2. Fill out all forms completely and to the best of your knowledge.  Please make sure that you have signed all forms that require a signature.  Any forms not completed or signed will be returned to you and will cause a delay in our interview process.  (Please note that the Medical Consent Form must be notarized.)

3. Turn in all application materials with the application, testing, registration and development fees.   

After reviewing the application materials, we will contact you for an interview and testing. 

Application Materials required:

1. Student Application Form

2. Medical Consent Form (Notarized)

3. Discipline Form

4. Student Confidential Reference Form*

5. Pastor's Reference Form*

6. Internet Usage Agreement Form

7. Blue State of Alabama Immunization Certificate (Original, not copy)

8. Copy of Student's Birth Certificate

*Student Confidential Reference and Pastor's Reference Forms are not in the application packet.  These forms will be filled out in the office when you turn in your completed Application Materials.
